Tesamorelin is a synthetic analog of human growth hormone-releasing hormone, consisting of the GHRH(1-44) sequence modified by the addition of a trans-3-hexenoic acid group at the N-terminus, which confers resistance to dipeptidyl peptidase-4 cleavage. It binds and activates GHRH receptors on pituitary somatotrophs, stimulating endogenous growth hormone release in a pulsatile pattern.
